As of August 26, 2003, initial registrations will be assessed a $200 late fee.
“WHO IS RESPONSIBLE TO SEE THAT I COMPLETE REQUIREMENTS FOR
MY PROGRAM OF STUDY?”
Ultimately, it is your responsibility to make sure you have
met all the requirements for your program of study. Regular, timely communication
with your faculty advisor is essential throughout your program
of study. Contact your advisor to discuss your progress and plan future
steps to meet requirements.

“HOW DO I CHANGE MY ADDRESS?”
Do one of the following, fax the request to 765-494-0570, or mail it to Office
of the Registrar, Purdue University, 1095 Hovde Hall, W. Lafayette, IN 47907-1095.
Include in the request the social security number, name, and address with
the new address, etc. BE SURE TO SIGN THE REQUEST.
